In the second chapter, where, Hester was forced to confess the name of her secret loveer in public, Arthur Dimmesdale places his hand over his heart '...Mr.Dimmesdale, who, leaning over the balcony, with his hand uponhis heart...' as if he is suffering from the same shame and guilt as Hester, but his suffering is a secret one.
And when the story progresses, Hawthorne shows that how Arthur becoming weak, and often under the attack of illnesses. Roger Chilingworth, the leech ( physician), with his own scheme of revenge comes to Arthur as a healer. Even he understands that Arthur has some thing in his mind that totally torments him. In the chapter 10 we could see that Chillingworth tells Arthur that as a physician he cannot cure him since his aliment seems to come from his spiritual side...
