Mr. Hector Luyando is a 52 year patient old Hispanic male, married, incapacitated swimming pool cleaner Chief Complaint: Mr. Hector had a pulsating and compressing chest pain that radiated directly to his back feeling a burning sensation in this area. Source: Patient (reliable historian?) HPI: 5 days ago Mr. Hector was hospitalized in San Pablo Hospital for an asthmatic attack and he recover and was sent to his home. 3 days later he came to the Emergency room complaining of the chest pain that he describe as a 10 being the maximum score and always wake him at night and Tylenol alleviated but not enough. He said his been having this chest pain for approximately 2 weeks ago without any recognizable precipitating events, and classify it as the worst pain he ever had. Other alleviating factor for his chest pain is sitting upright, no alleviation with nitroglycerine. He was asphyxiated with a sensation of breathlessness and also was found with hypertension. He reports that he’s being asthmatic since he was 35 years, some precipitating factors he recognized are cigarette smoke, gasoline fumes, climbing stairs or walking a short period of time, bending down, is always proceeded with an immediate reaction of obstruction in airways and a sense of hyperinflation in his chest.
