Present History: The client came into the hospital today with complaints a fatigue, nervousness, nausea, and feeling faint. The blood pressure was 112/67. Pulse was at 33 bpm. Temperature was 102.5. Clients weight is 186 lbs. The client stated that he was diabetic and had been taken his insulin as prescribed, yet his blood sugar level had remained high for over 1 week. Client denies any known infection to lower extremities. The wife stated that the client had been working hard in the field for the past 2 weeks and felt that this was a factor to his present health. No identifiable wounds noted to the feet or legs. The client states he has a beer or alcoholic beverage approximately 1-2 times a week. The client states that he has noticed his pants as being a little lose than they are usually. The client admits that he has had frequent trips to the bathroom at night and had excessive thirst for the past several day. Past History The client was diagnosed with late stage diabetes 10 months ago with late stage non-insulin dependent diabetes after a slow healing infection to his right great toe. 2 months post the diagnoses, the client was put on Humulin insulin because his blood sugar level remained uncontrollable with oral prescription for the diabetes. The client was also placed on for high blood pressure. The right great toe was amputated 1 month after the diabetic diagnosis due to the failure of the wound to heal and gangrene that overcame the great toe. The client was a heavy smoker for 30 years, but has quit since diagnosed with the diabetes.
