Chemical oxygen generator
On May 11th 1996 shortly after takeoff from the Miami Intl A/p Valuejet flight 592 crashed into the Florida Everglades. Both Pilots, the three flight attendants, and all 105 passengers were killed.
According to the investigation conducted by the NTSB - National Transportation Safety Board five cardboard boxes containing 144 chemical oxygen generators were loaded on the aircraft shortly before departure.
Chemical oxygen generators when transported as cargo, are considered a hazardous material and therefore must be properly packaged, labeled, and identified. These generators were shipped as company material and were not identified as hazardous material.
Approx. 12 min after take off one or more of the generators self activated casing a fire in the cargo area which quickly spread to the rest of the plane.
Today I would like to discuss the proper training of hazardous material and HCS compliance programs and the consequences for not complying with the hazardous material requirements...
